a) Booking System Information
Clients book sessions through a third-party booking platform used by Monarch Wellness & Yoga Services. The
booking system collects: Full name Phone number Email address This information is used solely for scheduling,
appointment management, and service-related communication. Clients are encouraged to review the booking
platform’s own privacy policy.
b) Client Intake Form Information
Clients must complete an intake form prior to receiving services. This form may collect: Full name Home address Date
of birth Email address Emergency contact name and phone number Reason for booking the session Current
medications Health-related information including diabetes, high blood pressure, or other conditions Major illnesses or
medical conditions Accidents or injuries Allergies Surgeries Broken bones Pregnancy status (yes or no) and how far
along Medical, health, or pregnancy-related issues (yes or no) Other therapies being used, excluding conventional
medicine
This information is collected to assess suitability for services, guide sessions safely, and respond appropriately in
emergencies.
2. Consent
By booking a session and submitting an intake form, you consent to the collection, use, and storage of your personal
information as outlined in this policy. Consent may be withdrawn with reasonable notice, subject to legal or contractual
limitations.
3. Use of Personal Information
Personal information is used for booking management, communication, session preparation, health and safety
considerations, and emergency response. Personal information is not used for marketing without explicit consent.
4. Disclosure of Information
Personal information is not sold, rented, or traded. Disclosure may occur only when required by law, to third-party
service providers for service delivery, or in emergencies to protect health or safety.
5. Storage and Safeguards
Personal information may be stored in both digital and physical formats. Safeguards include secure digital storage,
password-protected devices, limited access, and printed intake forms stored in a secure, locked location. Physical
records are handled with care and not left unattended in public spaces. Records are retained only as long as
necessary or as required by law.

Clients may request access to or correction of their personal information by submitting a written request. Requests will
be addressed within a reasonable timeframe.
7. Withdrawal of Consent
Consent may be withdrawn at any time. Withdrawal may limit or prevent the ability to provide services.

This Privacy Policy may be changed or updated at any time without prior notice. The most current version will be
